Program Specialist - Registry Program (Part-time) — June 5, 2008



DATE: June 5, 2008

POSITION: DRIVER (Per Diem)
RESPONSIBLE TO: Program Director, Hong Fook Adult Day Health Care Center
COMMITMENT (HOURS): On-call
STATUS: Non-Exempt (3 months orientation period)
APPLICATION DEADLINE: Open until filled

JOB SUMMARY: Under the direction of the Program Director of Hong Fook ADHC Center, the driver is responsible for transportation related services of Family Bridges, Inc. 

MAJOR DUTIES AND RESPONSIBILITIES:

  • Promptly and safely transport Family Bridges Inc.’s participants to and from the agency.  
  • Assist in the transportation of meals from the meal provider to the Center and the food containers back to the meal provider.  
  • Adhere to transportation requirements of our Auto Insurance company.
  • Complete the appropriate paper work related to the program accurately.
  • Assist in the transportation of office equipment and supplies.  
  • Provide transportation for field trips when needed.  
  • Maintain the vans in clean, safe and operable condition. 
  • Attend appropriate meetings, eg. safety committee meetings, staff meetings.
  • Attend all training sessions and in-services assigned by the Program Manager.
  • Perform other appropriate tasks deemed necessary by the Program Manager or Center Director.  

QUALIFICATIONS:

Possession of a California Class B driver’s license and safe driving record.

  • Will undergo driving test.
  • Physically capable to handling maximal assist participants with transfers.
  • Familiarity with Oakland Area.
  • Bilingual in English and an Asian Language preferred.
  • Experience with elderly / handicapped population preferred.

ADA REQUIREMENTS:   

Lifting of at least 50 lbs.

  • Good vision and hearing required for driving.
  • Ability to sit or stand for extended period.

 SALARY RANGE: Depends on Experience.

POSITION: MEDICAL SOCIAL WORKER, Bi-lingual Cantonese/English
RESPONSIBLE TO: Social Work Supervisor, Hong Fook Center
COMMITMENT (HOURS): Full-time (40 hours/week)
STATUS: Non-Exempt (3 months orientation period)

JOB SUMMARY: Under the supervision of the Social Work Supervisor, the Medical Social Worker is responsible for comprehensive psychosocial and case management services to the participants of Hong Fook Adult Day Health Care Center. The Medical Social Worker is also responsible for developing and implementing outreach activities to the community.

MAJOR DUTIES AND RESPONSIBILITIES:

  • Screens potential program participants by meeting with family members and referring agency representatives; completes intake assessments, including making home visits to prospective participants.
  • Provides psychosocial assessments of participants; develops and implements social work treatment plans.
  • Works with the Multidisciplinary Team to develop appropriate treatment plans for participants.
  • Maintains current, written case management records, including initial assessments, re-assessments and on-going evaluation of participants’ care.
  • Provides individual and family/caregiver counseling.
  • Develops and leads support groups for participants and their caregivers.
  • Refers participants and their families to appropriate community agencies or facilities; acts as liaison with such organizations and as an advocate for participants and/or their families.
  • Meets all administrative requirements, including preparing required reports and attending staff meetings.
  • Represents the Agency with human service, health care and community groups and individuals.  Maintains consistent, close contact with existing and potential referral sources.  Makes outreach presentations to professionals and community groups.
  • Other duties as assigned by Social Work Supervisor or Program Director.  

QUALIFICATIONS:

  • Master’s Degree from a School of Social Work (MSW) accredited by the Council on Social Work Education required.
  • Bilingual in Cantonese/English required.  Knowledge of Mandarin is a plus.
  • Minimum of 1-year experience working in a human services setting.
  • Experience in working with frail seniors and in a Multidisciplinary Team setting.
  • Possession of a valid California Driver’s License and vehicle to be used during home visits.
  • Experience in performing community outreach.

COMPETENCIES

  • Thorough knowledge of social work principles and practices, including case management, individual and group counseling techniques, and of community services and facilities which are available to seniors and their families.
  • Skills in preparing complete and concise written materials, in assessing and effectively meeting the needs of the participants and their families, and in working effectively with other team members, representatives of community organizations and the public.

ADA REQUIREMENTS: 

  • Ability to speak, see and hear clearly.
  • Ability to communicate effectively, written and oral.
  • Ability to use a computer.

 SALARY RANGE: $38.8K - $55.3K per annum, depending on experience, plus benefits

POSITION: PROGRAM SPECIALIST- Registry Program (Part-time)
RESPONSIBLE TO: Social Services Manager
COMMITMENT (HOURS): Part-time (30 hours/week)
STATUS: Non-Exempt (3 months orientation period)
APPLICATION DEADLINE: Open until filled

JOB SUMMARY: The Program Specialist, under the supervision of the Social Services Manager, will be responsible for the implementation of the In-Home Support Services (IHSS) Registry Program of Family Bridges, Inc. The program serves elderly and disabled persons from a centralized IHSS registry. IHSS consumers (clients) is able to access this program free of charge. For IHSS consumers, all attendant wages are subsidized by the county.

MAJOR DUTIES AND RESPONSIBILITIES:

  • Develop and maintain contacts with referral sources in order to increase consumer base.
  • Assist consumers to access the IHSS through application process.
  • Conduct outreach to potential consumers, other service providers and health care facilities in order to educate the community about the availability of the IHSS Registry.
  • Maintain positive relationship with consumers and their families in order to provide a high quality of care.
  • Recruit, screen, interview and maintain a pool; of qualified home care attendants sufficient to meet the needs of consumers.
  • Provide attendant referrals to IHSS registry consumers; assist with facilitating a match according to the needs of both the consumer and the attendant.
  • Maintain information in the Central Registry database.
  • Comply with current grant requirements and objectives.
  • Other duties as requested.

QUALIFICATIONS:

  • Appropriate educational background – BA in social services or business-related field.
  • Knowledge or experience in utilizing the resources of the East Bay health cares community.
  • At least two years of professional work experience.
  • Ability to work independently, as well as in a team.
  • Ability to be creative and adapt to new work environments and service demands.
  • Proficiency with word processing, spreadsheet and database software.
  • Experience working with elderly or working in a home health care setting.
  • Ability to communicate clearly in person and in writing.
  • Bilingual in Cantonese and English (both oral and written preferred).

  • Ability to drive.
  • Ability to use a computer.
  • Ability to communicate clearly with others.
  • Ability to lift of at least 20 lbs.

SALARY RANGE: $21.8K - $31.1K per annum (FTE), plus benefits
