FOR DOCTORS


Diabetes Self-Management Training (DSMT) promotes the successful self-management of diabetes by giving participants the knowledge, skills, and tools they need to take charge of their health and improve their clinical outcome and quality of life. Classes and individual interventions are offered in Cantonese, Mandarin, or English.

Our outpatient DSMT is given in an interactive group setting. Classes are taught by certified diabetes educators, registered dietitians and a registered nurse and other health care professionals who have completed specialized training in the management of diabetes and whose work is evidence-based. We abide by the professional code of ethics as set out by the American Association of Diabetes Educators (AADE) and the American Dietetic Association.

This program is one of four accredited DSMT programs in Alameda County by the American Diabetes Association (ADA). The comprehensive class curriculum covers ten content areas, as required by the ADA. Our classroom program provides one hour of individual assessment, and nine hours of group education. We place particular attention on excellent and detailed communication with our clients’ healthcare providers to facilitate coordinated and integrated care.

Client Eligibility for Services

To Enroll
1. Please fill out our diabetes services order form for your patient and fax it back to us at (510) 419-0880.
2. We will call your patient to schedule the initial assessment.

Initial Assessment
1. We ask your patient to bring all current medications and glucose meter.
2. We review and discuss the patient's current medication(s), eating style and physical activity, look at his/her blood glucose level, and medical history. We will also evaluate height, weight, and blood pressure.
3. We will teach the patient how to effectively monitor his/her blood sugar using the latest tools which can minimize discomfort and make the monitoring process easier.
4. We will direct the patient to attend an upcoming class or have an individual session, depending on the results of the initial assessment.
