FOR DOCTORS

Physicians can use Adult Day Health Care Services programs to complement their care for elders and adults with chronic health conditions such as:

  • Diabetes
  • Hypertension
  • Stroke
  • Heart conditions
  • Dementia
  • Ambulation limitations
  • Some behavioral conditions
  • Other chronic illnesses
Adult Day Health Care Services programs are state-licensed, daytime, community-based programs that combine health services and therapeutic activities in a structured environment that offers physical and cognitive stimulation and socialization.
An individualized assessment is conducted upon enrollment, and an individualized care plan is developed for each participant. A physician's report is required, and the physician can give standing orders for over-the-counter and emergency medications.
Staffing includes a registered nurse, social worker, physical therapist, occupational therapist and speech therapist, registered dietitian, psychiatric consultant, program director, activity director, program aides, rehabilitation aides, administrative personnel, and volunteers.

From Adult Day Services Program Support Physicians in Caring for Chronically Ill Patients, by Maureen Dixon, Alameda-Contra Costa Medical Association Bulletin, September 2007.
