FOR DOCTORS
Physicians can use Adult Day Health Care Services programs to complement
their care for elders and adults with chronic health conditions such as:
- Diabetes
- Hypertension
- Stroke
- Heart conditions
- Dementia
- Ambulation limitations
- Some behavioral conditions
- Other chronic illnesses
Adult Day Health Care Services programs are state-licensed, daytime, community-based
programs that combine health services and therapeutic activities in a structured
environment that offers physical and cognitive stimulation and socialization.
An individualized assessment is conducted upon enrollment, and an individualized
care plan is developed for each participant. A physician's report is required,
and the physician can give standing orders for over-the-counter and emergency
medications.
Staffing includes a registered nurse, social worker, physical therapist, occupational
therapist and speech therapist, registered dietitian, psychiatric consultant,
program director, activity director, program aides, rehabilitation aides, administrative
personnel, and volunteers.
