What services do you provide at the Hong Fook Adult Day Health Care Centers?

Our social, health and therapeutic services at the Hong Fook Centers include the following:

Nursing Care

  • Monitoring health status
  • Assistance with medications
  • Personal care (e.g. assistance with toileting and grooming)
  • Health education
  • Liaison with personal physician

Rehabilitation

  • Physical therapy

Nutrition

  • Nutritionally-balanced hot ethnic meal served daily

We also offer a nutritionally-balanced, hot ethnic meal and a nutritious snack every day.

Who is eligible for Hong Fook Adult Day Health Care services?
The Hong Fook Centers serve frail adults over 18 years of age who are medically eligible. There is no charge for Medi-Cal eligible participants and a sliding scale fee for private-pay individuals.

We serve frail adults who have the following conditions:

  • High blood pressure
  • Diabetes
  • Stroke
  • Cardiovascular conditions
  • Dementia
  • Any other condition that causes difficulty with living independently

How do I know if I'm eligible for services? How can I enroll?
For information on eligibility or enrollment, please call (510) 839-9673 or (510) 302-0460. During program hours, the receptionist will direct your call to the appropriate social worker who speaks your language. After hours, the multilingual message will direct you to the appropriate voicemail box of the social worker who speaks your language.

What languages do your staff members speak?
Our staff members speak Cantonese, Mandarin, Korean, and Vietnamese as well as English.

Do you provide transportation for program participants?
The Hong Fook Centers provide transportation for our participants (including wheelchair users) within: Oakland Chinatown, West Oakland, East Oakland, downtown Oakland and the City of Alameda. Whenever possible, families are encouraged to provide transportation.

What is the enrollment process?
1. A social worker will respond to your inquiry and will schedule you for a Center visit.
2. During your visit, the social worker will explain more about the program, give you a brief tour, and give you a packet of Center information.  
3. You will then be scheduled for a three-day assessment which will be used to develop a plan for your care.
4. Based on the assessment, we will make a recommendation about admission to the Center and the number of days to attend.
5. If you accept the invitation to attend, an enrollment date will be arranged.

The Hong Fook Centers are located within the historic Hotel Oakland. Hong Fook 14th Street Center: 275- 14th Street, Oakland, CA 94612 and Hong Fook Harrison Street Center: 1388 Harrison Street, Oakland, CA 94612.

Program Hours: Monday-Friday 7:30 a.m. to 5:30 p.m.
Session 1: 7:30 a.m. to 12:30 p.m.
Session 2: 12:30 p.m. to 5:30 p.m.
